Abstract
CILJEVI ISTRAŽIVANJA: Cilj istraživanja bio je utvrditi prediktivnu povezanost vrijednosti RDW–a (od engl. red cell distribution width, širina distribucije eritrocita) pri postavljanju dijagnoze MM-a (mutiplog mijeloma) s preživljenjem bolesnika s MM-om te istražiti prognostičku vrijednost broja eritrocita, trombocita i leukocita u tom istom vremenu za preživljenje ovih bolesnika. NACRT ISTRAŽIVANJA: Istraživanje je ustrojeno kao kohortno povijesno istraživanje. ISPITANICI I POSTUPCI: U istraživanje je uključeno 27 oboljelih od MM-a,liječenih na Odjelu za hematologiju, Klinike za Internu medicinu, Kliničko bolničkog centra Osijek u razdoblju od 2006. do 2012. godine. Iz medicinskih zapisa preuzeti su podaci o krvnim pretragama učinjenim pri postavljanju dijagnoze MM-a (RDW-a, leukocita i trombocita) kao i datuma postavljanja dijagnoze te datuma smrti bolesnika. Rezultati su analizirani deskriptivno i analitički, a statistički obrađeni pomoću SPSS-a (inačica 20,0). Preživljenje je analizirano Kaplan-Meirovim testom (Log-rank test). Razina statističke značajnosti bila je uz P < 0,05. REZULTATI: Povećane vrijednosti RDW-a pri postavljanju dijagnoze MM-a imale su negativnu prognostičku vrijednost i negativno utjecale na daljnju prognozu bolesti quoad vitam. Broj eritrocita pri postavljanju dijagnoze MM-a kao i broj leukocita nije bio prediktivan za preživljenje. Trombocitopenija kao i trombocitoza su se pokazale kao negativan prediktivni čimbenik u MM –u, granično statistički značajan. ZAKLJUČAK: Povećane vrijednosti RDW-a pri postavljanju dijagnoze MM-a imaju negativnu prognostičku vrijednost. Bolesnici s povećanim RDW-om pri postavljanju dijagnoze MM-a imaju kraće preživljenje.
Abstract (english)
AIMS: The aim of the study was to determine predictive relationship of RDW (red cell distribution width) at the time of MM (multiple myeloma) diagnosis with patients' survival and to investigate predictive values of number of red blood cells (RBC), white blood cells (WBC) and platelets at that time point for patients' survival. DESIGN: The study was historical cohort study. PATIENTS AND METHODS: Study included 27 MM patients treated at Department for Hematology, Internal Clinic, University Hospital Centre Osijek, during the period from 2006 until 2012. The dana on blood findings at the time of MM diagnosis (RDW, RBC, WBC, platelets), the dates of diagnosis and death were taken from medical records. The results were analyzed descriptively and analytically with SPSS statistical programm (version 20.0). Survival was analyzed by Kaplan-Meier test (Log-rank). Statistical difference was considered at P < 0.05. RESULTS: Increased RDW at the time of MM diagnosis negatively predicted the disease prognosis. RBC and WBC were not predictive for the survival time. Marginally significant predictive value for survival was found for pathologically decreased and increased platelets count, respectively. CONCLUSION: Increased RDW at the time of MM diagnosis has negative predictive value. Patients with increased RDW had shorter survival.
