Abstract | Cilj istraživanja: Cilj ovog istraživanja je bio procijeniti udjele transplantiranih bolesnika s obzirom na tehniku izvođenja zahvata, indikacije za zahvat i potrebu za reoperacijom tijekom razdoblja od 2013. − 2023. godine, te ispitati postoji li razlika u ishodima s obzirom na tehniku izvođenja zahvata.
Nacrt studije: Presječno istraživanje s povijesnim podatcima.
Ispitanici i metode: U studiju su bili uključeni pacijenti, kojima je transplantirana amnijska membrana na Klinici za očne bolesti Kliničkog bolničkog centra Osijek, u razdoblju od 2013. do 2023. godine. U istraživanju su se koristili medicinski zapisi koji sadrže podatke o indikaciji za postupak transplantacije hAM, vidnoj oštrini prije i poslije zahvata, te eventualnoj reoperaciji, demografskim obilježjima (dob, spol), te tehnici transplantacije amnijske membrane. Ti su podatci statistički obrađeni i uspoređeni.
Rezultati: Istraživanje je provedeno na 69 pacijenata, medijana dobi od 77 godina. Najučestalija indikacija za transplantaciju amnijske membrane bila je bulozna keratopatija kod 52 (75,4 %), zatim ulkus rožnice kod 7 (10,1 %) i perforacija rožnice kod 5 (7,2 %) ispitanika. Većina pacijenata imala je jednoslojni zahvat, njih 40 (58 %). Reoperacija je bila potrebna kod šest pacijenata (8,7 %). Nije pronađena značajna povezanost između vrste zahvata i potrebe za reoperacijom (Fisherov egzaktni test, P = 0,24). Vidna oštrina pacijenata značajno se poboljšala nakon zahvata (Wilcoxonov test, P < 0,001).
Zaključak: Transplantacija amnijske membrane se pokazala učinkovitom metodom liječenja različitih patoloških stanja rožnice, iako je kod manjeg broja pacijenata bila potrebna reoperacija. Nije pronađena značajna povezanost između vrste zahvata i učestalosti reoperacija. |
Abstract (english) | Title: Amniotic membrane transplantation in the treatment of anterior segment eye diseases
Objectives: The objectives of this study were to evaluate the distribution of transplanted patients based on the surgical technique used, the indications for the procedure, and the need for reoperation during the period from 2013 to 2023, as well as to examine whether there are differences in outcomes depending on the surgical technique used.
Study Design: A cross-sectional study with historical data
Patients and Methods: The study included patients who underwent amniotic membrane transplantation at the Department of Ophthalmology, University Hospital Center Osijek, during the period from 2013 to 2023. The study utilized medical records that contain data on the indication for the hAM transplantation procedure, visual acuity before and after the procedure, any potential reoperation, demographic characteristics (age, gender), and the technique used for amniotic membrane transplantation. These data were statistically analyzed and compared.
Results: The study was conducted on 69 patients with a median age of 77 years. The most frequent indication for amniotic membrane transplantation was bullous keratopathy in 52 patients (75.4%), followed by deep corneal ulcers in 7 patients (10.1%), and corneal perforation in 5 patients (7.2%) The majority of patients, 40 (58%), underwent a single-layer procedure. Reoperation was necessary in six patients (8.7%). No significant association was found between the type of procedure and the need for reoperation (Fisher's exact test, P = 0.24). The patients' visual acuity significantly improved after the procedure (Wilcoxon test, P < 0.001).
Conclusion: Amniotic membrane transplantation has been shown to be an effective method for treating various corneal pathologies, although a small number of patients required reoperation. No significant association was found between the type of procedure and the frequency of reoperations. |
