Abstract Cilj istraživanja: Cilj istraživanja bio je procijeniti psihofizičko stanje sudionika nefatalnih cestovnih prometnih nesreća i čimbenike povezane s lošijim ishodima mentalnoga zdravlja i kvalitete života nakon cestovne prometne nesreće. Nacrt studije: Kohorta od 200 ispitanika prospektivno je praćena tijekom 6 mjeseci nakon preživljene cestovne prometne nesreće. Ispitanici i metode: Ispitanici su bili sudionici cestovnih prometnih nesreća sa svim težinama ozljeda te neozlijeđeni sudionici cestovnih prometnih nesreća. Jedan mjesec i šest mjeseci nakon cestovne prometne nesreće procijenjena je kvaliteta života ispitanika i prisutnost simptoma posttraumatskoga stresnog poremećaja, depresije i anksioznosti s pomoću standardiziranih samoocjenskih upitnika. Analizirana je povezanost kvalitete života i simptoma psihičkih poremećaja sa sociodemografskim čimbenicima, zdravstvenim stanjem prije i poslije cestovne prometne nesreće, ozljeđivanjem u nesreći i čimbenicima povezanima s nesrećom. Rezultati: Prospektivno praćena kohorta sudionika cestovnih prometnih nesreća tijekom promatranoga razdoblja karakterizirana je značajnim udjelom sudionika sa simptomima posttraumatskoga stresnog poremećaja (35,5 % i 20,5 % nakon 1 i 6 mjeseci) i depresije (20,0 % i 13,5 % nakon 1 mjeseca i 6 mjeseci) te niskim udjelom ispitanika sa simptomima anksioznosti (4,5 % i 3,5 % nakon 1 mjeseca i 6 mjeseci). Istraživanjem je ustanovljena povezanost ozljeđivanja i težine ozljeda s nižim socioekonomskim statusom, zdravstvenim stanjem prije nesreće i čimbenicima povezanima s nesrećom. Ustanovljena je povezanost simptoma posttraumatskoga stresnog poremećaja i depresije sa sociodemografskim čimbenicima, zdravstvenim stanjem prije i poslije nesreće, ozljeđivanjem u nesreći i čimbenicima povezanima s nesrećom. Simptomi anksioznosti povezani su sa zdravstvenim stanjem sudionika prije i poslije nesreće i vrstom sudionika u prometu, a ne s ozljeđivanjem u nesreći. Kvaliteta života sudionika cestovnih prometnih nesreća povezana je sa sociodemografskim čimbenicima, zdravstvenim stanjem prije i poslije nesreće, ozljeđivanjem u nesreći, čimbenicima povezanima s nesrećom te sa simptomima psihičkih poremećaja nakon nesreće. Zaključak: Identifikacija čimbenika povezanih s lošijim ishodima mentalnoga zdravlja i kvalitete života nakon cestovne prometne nesreće omogućuje probiranje sudionika cestovnih prometnih nesreća rizičnih za loš oporavak nakon nesreće. Svim sudionicima cestovnih prometnih nesreća, neovisno o ozlijeđenosti, treba procijeniti zdravstveno stanje prije nesreće i druge čimbenike povezane s lošijim oporavkom. Procjena i bilježenje kvalitete života trebali bi biti dio rutine zdravstvene skrbi za sudionike cestovnih prometnih nesreća.
Abstract (english) Objectives: The aim of the study was to explore psychophysical condition of the survivors of non-fatal road traffic accidents and factors associated with poor mental health outcomes and low quality of life after experiencing a road traffic accident. Study design: A cohort of 200 participants was followed prospectively during the period of 6 months following a road traffic accident. Participants and methods: Participant were the victims of a road traffic accidents with all injury severity levels, as well as uninjured survivors of a road traffic accidents. Quality of life of the participants and the presence of symptoms of posttraumatic stress disorder, depression and anxiety were assessed 1 month and 6 months following a road traffic accident. Association between the quality of life and the symptoms of psychiatric disorders, and sociodemographic factors, health status before and after the road traffic accident, injury-related and accident-related factors was analysed. Results: Prospectively followed cohort of a road traffic accident survivors was characterized with significant proportion of the participants with the symptoms of posttraumatic stress disorder (35,5% and 20,5% after 1 and 6 months) and depression (20,0% and 13,5% after 1 and 6 months), and with low proportion of the participants with anxiety symptoms (4,5% and 3,5% after 1 and 6 months). Sustaining injury and injury severity were associated with lower socioeconomic status, previous health status and accident-related factors. Symptoms of posttraumatic stress disorder and depression were associated with sociodemographic factors, health status before and after the road traffic accident, injury-related and accident-related factors. Anxiety symptoms were associated with health status before and after the road traffic accident and the type of road user, but not with injury-related factors. Quality of life after the road traffic accident was associated with sociodemographic factors, health status before and after the road traffic accident, injury-related and accident-related factors, and with the symptoms of psychiatric disorders after the road traffic accident. Conclusion: Identification of factors associated with poor mental health outcomes and low quality of life following a road traffic accident enables screening of individuals at risk of poor recovery after a road traffic accident. All road traffic accident survivors, irrelevant of their injury status, should be assessed for their previous health status and other factors associated with poor recovery. Quality of life assessment and recording should be a part of routine healthcare for the road traffic accident survivors.
