Abstract | Cilj istraživanja: Cilj ove studije je ispitati je li vrijednost omjera C- reaktivnog proteina (CRP) i albumina u vrijeme utvrđivanja dijagnoze neovisni prognostički čimbenik ukupnoga preživljenja i odgovora na terapiju u bolesnika s multiplim mijelomom (MM) te ispitati međusobni odnos između omjera CRP i albumina u vrijeme utvrđivanja dijagnoze u bolesnika s MM, kao i njegovu povezanost s demografskim, kliničkim i laboratorijskim obilježjima (spol, dob, vrijednost eritrocita (E), apsolutna vrijednost leukocita (L), apsolutna vrijednost limfocita, apsolutna vrijednost trombocita (Trc), serumska koncentracija hemoglobina (Hgb) i vrijednost laktat-dehirdogenaze (LDH), postotkom plazma stanica u koštanoj srži, serumskom vrijednosti kalcija, kreatinina i β-2-mikroglobulina). Nacrt studije: Studija je oblikovana kao presječna studija s povijesnim podatcima. Ispitanici i metode: U istraživanju je sudjelovalo 24 pacijenta s dijagnozom MM u periodu od 01. 12. 2017. do 01. 01. 2021. na Zavodu za hematologiju Kliničkoga bolničkoga centra Osijek. Prikupljeni su osnovni anamnestički podatci (dob, spol, datum dijagnoze), osnovni laboratorijski podatci s datumom postavljanja dijagnoze (Hgb, hematokrit, E, L, limfociti, MPV, RDW, LDH, kalcij, Trc, CRP, albumin, β-2-mikroglobulin) te podatci citološke punkcije (% plazma stanica u koštanoj srži, tip multiplog mijeloma, elektroforeza proteina) iz povijesti bolesti. Rezultati: Dobiveno je da nema značajnih razlika u vrijednosti omjera CRP i albumina s obzirom na spol i ishod liječenja. Također, dobiveno je da nema značajnih razlika u vrijednosti omjera CRP i albumina s obzirom na odgovor na terapiju te da nema statistički značajne povezanosti između omjera CRP i albumina s dobi i prikazanim biokemijskim pokazateljima. Zaključak: Omjer CRP i albumina u bolesnika s dijagnozom MM pri postavljanju dijagnoze nema statistički značajnu povezanost s ukupnim preživljenjem i odgovorom na terapiju. Abstract (english) | Objectives: The aim of this study was to examine whether the ratio of C-reactive protein (CRP) to albumin at the time of diagnosis was an independent prognostic factor for overall survival and response to therapy in patients with multiple myeloma (MM) and to examine the relationship between CRP and albumin at the time of diagnosis in patients with MM and its association with demographic, clinical and laboratory characteristics (sex, age, erythrocyte value (E), absolute value of leukocytes (L), absolute value of lymphocytes, absolute value of platelets (Trc), serum hemoglobin concentration (Hgb) and lactate dehydrogenase (LDH), bone marrow plasma cell percentage, serum calcium, creatinine, and β-2-microglobulin). Study Design: The study is designed as a cross-sectional study with historical data. Patients and Methods: The study involved 24 patients diagnosed with MM in the period from 01. 12. 2017. to 01. 01. 202.1 at the Department of Hematology of the Osijek University Hospital. Basic anamnestic data (age, sex, date of diagnosis), basic laboratory data with date of diagnosis were collected (Hgb, hematocrit, E, L, lymphocytes, MPV, RDW, LDH, calcium, Trc, CRP, albumin, β-2- microglobulin) and cytological puncture data (% plasma cells in bone marrow, multiple myeloma type, protein electrophoresis) from medical history. Results: It was found that there were no significant differences in the value of the CRP to albumin ratio with respect to gender and treatment outcome. Also, it was found that there were no significant differences in the value of the CRP to albumin ratio with respect to response to therapy, and that there was no statistically significant association between the CRP to albumin ratio with age and the presented biochemical parameters. Conclusion: CRP to albumin ratio in patients diagnosed with MM at diagnosis has no statistically significant association with overall survival and response to therapy. The CRP to albumin ratio in patients diagnosed with MM at diagnosis has no statistically significant association with demographic, clinical, and laboratory characteristics. |
