Outcomes of hypersensitivity testing to intravenous anesthetics and neuromuscular blockers Objectives: The aim of this study was to analyze the percentage of subjects with positive skin tests and to evaluate the relationship between the results of intravenous anaesthetics and neuromuscular blockers hypersensitivity testing and the demographic characteristics, existing comorbidities, severity of previous reactions, the time interval from last reaction, and the presence of hypersensitivity to other allergens. Study design: The study was conducted as a cross-sectional study. Subjects and methods: The subjects were patients tested for hypersensitivity to intravenous anaesthetics and neuromuscular blockers in Clinical Pharmacology outpatient clinic at the Clinic for Internal Diseases as a part of Clinical Hospital Center in Osijek, in the period from early October 2016 to the end of December 2020. The total number of subjects was 50, and the data collected from the medical records of each patient included demographic data, existing comorbidities, data on hypersensitivity to other allergens, data on perioperative hypersensitivity reactions, and outcomes of prick and intradermal testing on intravenous anaesthetics and neuromuscular blockers. The collected data were analyzed in relation to the outcome of testing. Results: There was a higher percentage of positive intradermal tests for the tested drugs compared to the prick tests, and the highest number of positive intradermal tests for rocuronium. A significant association was found between: a positive prick test on vecuronium and hypersensitivity to nutritional allergens; positive skin tests on etomidate and autoimmune diseases; a positive prick test on etomidate and a recorded perioperative airway spasm, and also between a positive intradermal test on fentanyl and recorded perioperative anaphylaxis. Other observed variables did not show a significant difference in relation to the outcome of testing. Conclusion: This cross-sectional study confirmed that neuromuscular blockers are more common causes of perioperative hypersensitivity reactions compared to intravenous anesthetics. Although a significant correlation was found between certain positive skin tests and some of the observed variables, due to the small number of subjects, clinical significance of the obtained results is uncertain.
