Objectives: To determine the incidence of twin pregnancies in the period of 2019. – 2021., to examine the existence of risk factors for twin births and their neonatal outcome. Compare this data in both twins, examine whether there are increased risks of such pregnancies in mothers and differences of the stated during the study period. Study design: Study of pairs with historical data Patients and methods: The research includes 284 children born from twin pregnancies and 284 children from control group born from singleton pregnancies, during 2019., 2020. and 2021. in the Clinical Hospital Centre Osijek. Mothers of twins have been compared to mothers of children from singleton pregnancies. Data was obtained from medical records. Category data is presented in absolute and relative frequencies. Statistical significance was accepted for p <0.05. Results: The incidence of twin pregnancies in 2019. was 22/1000, in 2020. 24/1000 and 25/1000 in 2021. IVF and emergency caesarean section are more common in twin pregnancies. Abnormal presentation dominates among twins, and the most common complication of the control group, at birth, is premature rupture of the amniotic sac. Twins have a lower gestational age, birth weight and length. Fetal asphyxia and lethal outcome are also more common. There are no significant differences between the first-born and the second-born twin, apart from the fact that in the first-born twin a disorder of amniotic fluid is more frequent, and in the second-born twin abnormal presentation is more frequent. The most common gestational disease is gestational anemia, more common in mothers of twins. During 2021., significantly more SARS-CoV-2 infections were recorded in the mothers of twins, and in the twins themselves the lethal outcome was more frequent in the same year. Conclusion: Twin pregnancies are accompanied by continuous risk, so antenatal care is key to reducing poor prognosis.
