Abstract
Cilj istraživanja: Primarni cilj istraživanja bio je ispitati utjecaj PV/INR na ishod nekardioembolijskog ishemijskog moždanog udara. Ostali ciljevi bili su: ispitati povezanost PV/INR s ishodom i težinom ishemijskog moždanog udara u osoba kod kojih je primijenjena revaskularizacijska terapija, a u svrhu liječenja ishemijskog moždanog udara te ispitati postoje li razlike u profilu PV-a osoba s akutnim ishemijskim moždanim udarom prije i za vrijeme COVID-19 pandemije. Nacrt studije: Napravljeno je presječno istraživanje i korišteni su povijesnih izvora. Ispitanici i metode: U istraživanje su uključeni svi bolesnici s akutnim moždanim udarom, stariji od 18 godina, koji su hospitalizirani zbog akutnog nekardioembolijskog ishemijskog moždanog udara na Zavodu za cerebrovaskularne bolesti i intenzivnu neurologiju i Klinici za neurologiju Kliničke bolnice „Sveti Duh“, u razdoblju od 1. siječnja 2018. do 31. prosinca 2021. godine. Rezultati: Značajno su niže vrijednosti PV-a, odnosno više vrijednosti INR-a i D-dimera (p < 0,001) u skupini ispitanika s lošim ishodom i težom kliničkom slikom prigodom prijema. Nisu zamijećene značajne razlike u pogledu PV/INR-a glede revaskularizacijskih procedura. Prije COVID-19 pandemije značajno su niže bile vrijednosti PV-a (p = 0,01) i D-dimera (p = 0,04) u odnosu na vrijeme pandemije. Zaključak: Niže vrijednosti PV-a, odnosno više vrijednosti INR-a i D-dimera povezane su s težom kliničkom slikom i lošijim ishodom nekardioembolijskog moždanog udara.
Abstract (english)
Objectives: The primary objective of this study is to examine the impact of prothrombin time/international normalized ratio (PT/INR) on the outcome of non-cardioembolic ischemic stroke. Other objectives are to examine the relationship of PT/INR on outcome and severity of ischemic stroke in subjects who received revascularization therapy and to examine whether there are differences in the PT for stroke subjects before and during the COVID-19 pandemic. Study design: Cross-sectional research using historical sources. Subjects and methods: The research included all acute stroke patients over the age of 18 who were hospitalized for acute non-cardioembolic ischemic stroke at the Stroke and Intensive Care Unit and the Department of Neurology of the Sveti Duh University Hospital in the period from 1st January 2018. until 31st December 2021. Results: Significantly lower values of PT and higher values of INR and D-dimer (p < 0,001) were observed in the group of subjects with a worse outcome and with a severe clinical picture (p < 0,001), Before COVID-19 pandemic, PT values (p = 0,01) and D-dimer values (p = 0,04) were significantly lower than during COVID-19 pandemic than during COVID-19 pandemic. Conclusion: Lower values of PT, and higher values of INR and D-dimer are associated with a worse outcome and more severe clinical picture of non-cardioembolic stroke.
