Abstract
CILJEVI ISTRAŽIVANJA: Ciljevi ovog istraživanja bili su ispitati značenje HLA tipizacije u dijagnostici aksijalnog spondiloartritisa, ispitati učestalost HLA B27 s obzirom na spol pacijenata, ispitati povezanost kliničkih simptoma s pojedinim HLA alelima te ispitati učestalost korištenja glukokortikoida i biološke terapije s obzirom na HLA tipizaciju. NACRT STUDIJE: Presječna studija s povijesnim podacima. ISPITANICI I METODE: U istraživanje su uključeni pacijenti oboljeli od aksijalnog spondiloartritisa, oba spola te svih dobnih skupina, koji su u razdoblju od 1. siječnja 2017. do 31. prosinca 2022. godine liječeni na Zavodu za reumatologiju, kliničku imunologiju i alergologiju Kliničkog bolničkog centra Osijek. Podaci za ovo istraživanje preuzeti su iz arhive medicinske dokumentacije Kliničkog bolničkog centra Osijek pomoću Bolničkog informacijskog sustava. Bilježeni su podaci o dobi i spolu pacijenata, terapiji, HLA tipizaciji te ostali klinički podaci. REZULTATI: Istraživanje je provedeno na 101 pacijentu oboljelom od aksijalnog spondiloartritisa, medijana dobi 56 godina u rasponu od 21 do najviše 80 godina. 45 (45 %) pacijenata su muškarci, a 56 (55 %) žene. U ispitanoj populaciji, HLA B27 je najučestaliji alel (kod 63 pacijenata (62 %)) i to značajnije češći kod muškaraca (80 %), dok kod drugih HLA alela nema značajnih razlika s obzirom na spol pacijenata. HLA B27 pacijenti znatno češće koriste glukokortikoide u svojoj terapiji, dok HLA B27 negativni pacijenti znatno češće koriste csDMARD-ove. Sakroileitis je uočen kod znatno više HLA B27 negativnih ispitanika. ZAKLJUČAK: Veći broj oboljelih je HLA B27 pozitivan, što je važan čimbenik u dijagnostici aksijalnog spondiloartritisa. HLA B27 znatno je učestaliji kod muškaraca nego kod žena. Pacijenti pozitivni na HLA B27 znatno češće koriste glukokortikoide i inhibitore TNF-α, a značajno više HLA B27 negativnih pacijenata koristi csDMARD-ove. Najveći broj pacijenata sa sakroileitisom su HLA B27 negativni.
Abstract (english)
OBJECTIVES: The objectives of this study were to examine value of HLA typing in diagnosing axial spondyloarthtitis, to examine frequency of HLA B27 in each sex, to examine association between clinical symptoms and HLA allels and to examine frequency of using biological therapy and glucocorticoids in different HLA allels. STUDY DESIGN: Cross-sectional study with historical data. PARTICIPANTS AND METHODS: The study included patients with axial spondylitis both sexes and all age groups, who were treated at the Department of Rheumatology, Clinical Immunology and Allergology in Clinical Hospital Center Osijek, from 1st of January 2017 to 31st of December 2022. Data for this study were collected from Clinical Hospital Center Osijek archive using BIS. Analysed data were including age, sex, therapy, HLA typing and other clinical information. RESULTS: The research was conducted on 101 axial spondylitis patients, median age 56 years ranging from 21 to a maximum of 80 years. 45 (45 %) patients are men, and 56 (55 %) are women. In the examined population, HLA B27 is the most frequent allele (in 63 patients (62 %)) and significantly more frequent in men (80 %), while there are no significant differences in the other HLA alleles with regard to the gender of the patients. HLA B27 patients use glucocorticoids much more often in their therapy, while HLA B27 negative patients use csDMARDs much more often. Sacroiliitis was observed in significantly more HLA B27 negative subjects. CONCLUSION: The majority of patients are HLA B27 positive, which is an important factor in the diagnosis of axial spondyloarthritis. HLA B27 is significantly more common in men than in women. HLA B27 positive patients use glucocorticoids and TNF-α inhibitors significantly more often, and significantly more HLA B27 negative patients use csDMARDs. The most of patients with sacroiliitis are HLA B27 negative.
