Abstract
Cilj istraživanja: Procijeniti pojavnost i opseg te karakterizaciju degenerativnih promjena slabinske kralježnice i zahvaćenost kralježaka, odnosno intervertebralnih diskova pojedinim promjenama te ispitati njihove dobne i spolne razlike. Nacrt studije: Presječna studija s povijesnim podatcima. Ispitanici i metode: Uključeno je 250 ispitanika, od toga 146 žena i 104 muškarca. Ispitanici su podijeljeni u 4 dobne skupine (grupa I: manje od 29 godina, grupa II: od 30 do 49 godina, grupa III: od 50 do 69 godina i grupa IV: više od 70 godina) koji su od 1. siječnja 2019. do 30. travnja 2019. bili na magnetskoj rezonanciji lumbalne kralježnice na uređaju jačine 3 Tesla. Za potrebe istraživanja koristili su se podatci iz PACS-a Kliničkog zavoda za dijagnostičku i intervencijsku radiologiju KBC-a Osijek. Rezultati: Različite degenerativne promjene otkrivene su kod svih ispitanika, više kod žena nego muškaraca. Velik broj (87,2 %) imao je desikaciju I.V. diska, 76,4 % bulging diska, 69,2 % fasetnu artrozu, a 64,4 % neki od oblika hernijacije diska. Znatan broj ispitanika ima degenerativne promjene na dva ili više I.V. diskova. Razine lumbalne kralježnice najzahvaćenije promjenama su L4-L5 i L5-S1. Zaključak: Degeneracija I.V. diska lumbalne kralježnice česta je pojava, a pojavnost joj raste s dobi. MR je koristan i preporučuje se u klasifikaciji lezija kralježnice, što pak utječe na modalitete liječenja i klinički ishod.
Abstract (english)
Objectives: The aim of this study was to evaluate the incidence, characterization, extent and changes associated with degenerative spondyloarthritis by magnetic resonance imaging, as well as to examine the differences relative to sex and age. Study Design: Cross-sectional study. Participants and Methods: The study included 250 participants, divided into 4 groups based on age (Group I: less than 29, Group II: 30 – 49 years, Group III: 50 – 69 years, and Group IV: more than 70 years), all of whom were diagnosed with some form of lumbar degenerative disease in a 4 month period (January 1st to April 30th 2019) at the Clinical Hospital Center Osijek, having undertaken magnetic resonance imaging using a 3 Tesla MR-machine. The data was extracted from PACS (Picture Archiving and Communication System). Results: Participants were affected with various forms of spinal degenerative diseases, females more so than males. I.V. disc desiccation was most common (87.2%), followed by disc bulging (76.4%), facet joint arthropathy (69.2%), and some form of disc herniation was present in 64.4% of participants. Levels L4-L5 and L5-S1 were most affected. Conclusion: I.V. disc degeneration of the lumbar spine is common, and its incidence rises alongside age. MRI is useful in classifying spinal lesions, which in turn affects the treatment and accompanying outcomes.
