Introduction: Erythrocyte sedimentation rate is a non-specific indicator of the presence of acute and chronic inflammatory conditions in human body. The automatized method on erythrocyte sedimentation device iSED Alcor Scientific can quickly get results. It is therefore important to validate this method and allow its everyday application. Objectives: The goal is to evaluate and compare the values of erythrocyte sedimentation rate obtained on the iSED Alcor Scientific automatic device with the values obtained by the standard laboratory method, and establish the effectiveness of the investigated methods for determining sedimentation rate. Study Design: Cross-sectional study. Material and Methods: Whole blood samples from 50 patients were used in the study. Then erythrocyte sedimentation rate analyses were performed. For the Westergren method, venous blood was collected in Vacutainer tubes that contained anticoagulant sodium citrate, and for analysis on the automatic iSED Alcor Scientific device in EDTA anticoagulant tubes. The obtained results were processed using the MedCalc computer program. Results: Comparing the results, obtained by analyzing the erythrocyte sedimentation rate using standard laboratory methods and automated methods on iSED Alcor Scientific Cusum test linearity, a good linear relationship of method (P = 0,100) is evident. Passing Bablok regression method did not find any statistically relevant differences, neither in systematic or proportional form, between the observed methods. Spearman's correlation coefficient is high (r = 0.856) and the correlation coefficient significance is P < 0.0001 and therefore, it is proven that the methods are mutually comparable and replaceable. Conclusion: The iSED Alcor method for determining the erythrocyte sedimentation rate showed excellent congruence with the standard laboratory method, meaning the methods are replaceable and compatible.
